目的:分析CYP2C19*3基因多态性与癫痫患者奥卡西平(oxcarbazepine,OXC)活性代谢产物10,11-二氢-10-羟基卡马西平(monohydroxycarbazepine,MHD)血药浓度的相关性。方法:纳入120例OXC单药治疗1个月以上且症状控制良好的癫痫患者,采集清晨服药前空腹血,采用高效液相色谱法测定MHD稳态谷浓度。通过PCR和sanger测序判定患者CYP2C19*3基因型。结果:120例癫痫患者快代谢患者64例,MHD血浆浓度为(18.17±7.34)μg·mL-1;中代谢型患者36例,MHD血浆浓度为(19.31±9.17)μg·mL-1;慢代谢型患者20例,MHD血浆浓度为(25.79±7.51)μg·mL-1,3种基因型的MHD浓度有显著性差异(F=7.077,P=0.0013)。多因素分析显示,日剂量作为影响血药浓度的重要指标呈现出显著相关性(P<0.05)。结论:CYP2C19*3基因多态性影响MHD血药浓度,并且日剂量越高,MHD血药浓度越大,两者成显著正相关,临床应进行血药浓度监测。 相似文献

BackgroundThis study aimed to assess the baseline levels of D-dimer, C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR) and monitor the natural course of these serum markers after uneventful primary total joint arthroplasty.MethodsThis prospective study enrolled 81 patients undergoing primary total knee arthroplasty or total hip arthroplasty. The level of serum D-dimer, CRP, and ESR was measured preoperatively and on postoperative days 1, 3, 5, 15, and 45. Mean peak values, peak times, and distribution were compared between D-Dimer, CRP, and ESR.ResultsThe mean preoperative serum D-dimer, CRP, and ESR level was 412 ± 260 (range 200-980) ng/mL, 2.93 ± 2.1 (range 1-18) mg/L, and 22.88 ± 17.5 (range 3-102) mm/h, respectively. The highest mean peak for D-dimer, CRP, and ESR was at postoperative day 1, 3, and 5, respectively.ConclusionD-dimer levels reached peak levels on postoperative day 1 and then declined rapidly to a plateau level by postoperative day 3. A second, albeit small, peak in the level of D-dimer occurred on postoperative day 15. The discovery of hepatitis C has been a landmark in public health as it brought the opportunity to save millions of lives through the diagnosis, prevention and cure of the disease. The combined work of three researchers, Alter H, Houghton M and Rice C, which set the basis for the diagnosis, treatment and prevention of hepatitis C apart from laying the ground work for a new approach to study infections in general and developing new antiviral agents. This is a story of a transfusion-associated infection. A series of clinical studies demonstrated the existence of an infectious agent associated with hepatitis. That was followed by the identification of what was later known to be the hepatitis C virus (HCV) and the development of diagnostic tests. It all preceded the full molecular identification and demonstration of a causal effect. Finally it ended up with the development and discovery of a new class of therapeutic drugs, the direct acting antivirals, which are now used not only to cure the disease but most probably, to eliminate the problem. This work started with Dr Alter H who demonstrated that a new virus was responsible for the majority of post-transfusion hepatitis followed by Houghton M who cloned the virus and developed the blood test to identify those cases that carried the virus.
